Community Health Nursing Assessment
Introduction
Community health nurses (CHNs) are professional nurses who work in collaboration with the community to improve their health. CHNs work closely with other members of the health care team, including physicians, dentists, and pharmacists; they also collaborate with social workers and other mental health professionals when appropriate. Community health nurses provide support to patients by providing education about lifestyle choices that can promote good health or prevent disease; by encouraging healthy lifestyles through public awareness campaigns; by referring people to resources that make it easier for them to achieve their desired level of physical activity; and through direct patient-centered care strategies such as helping patients identify barriers that prevent them from taking preventive actions such as quitting smoking or following a healthy diet plan.”
