• Describe your assigned client’s situation. What holistic assessment data concerns you?
• Review the 5M framework elements from this week’s lesson. How does the 5M framework apply to your particular client? 
• Consider the client’s current presentation. What plan of care would you recommend and why?
• Consider the provider’s role in delivering culturally sensitive care, eliminating health disparities, fostering health promotion and disease prevention, and preventing polypharmacy. What strategies will you use to address at least one of these concerns in your client’s situation?

CLIENT’S CASE:

Alice Lightfoot, a 76-year-old Indigenous client with multiple comorbidities, including chronic kidney disease, hypertension, hyperlipidemia, and type 2 diabetes mellitus, is facing significant challenges in managing her health. She lives alone, requires outpatient hemodialysis three times a week, and takes at least 15 daily medications. Recently, Alice missed a dialysis appointment due to transportation issues, highlighting a critical barrier to her care. Financially, she is struggling despite being on Medicaid and Medicare, as the high cost of her medications leaves her unable to afford groceries. She is also having a difficult time finding transportation to the grocery store. She has tried walking a few times but tires quickly. Emotionally, Alice is exhausted and depressed, feeling overwhelmed by her situation and unsure of what to do next.
