The purpose of the Collaborative Learning Community is to enhance the quality of life for individuals with chronic illness and their families.

The goals are to:

  • Build community through education, sharing and support.

  • Provide opportunities for individuals with chronic illness or caregivers who have an acute illness or condition.

  • Facilitate collaboration between health care providers, schools, employers and other organizations by providing resources that promote wellness in all community members including those with ongoing health issues such as Alzheimer’s disease (AD) or Parkinson’s Disease (PD).

In addition to knowing what those spiritual needs are, it also allows you to better understand how they fit into your role as a healthcare provider or nurse. It will allow you to make informed decisions about when it’s appropriate for them seek outside help or if they should be referred directly back into their home environment (if this is possible).
