As a healthcare professional, understanding where to locate evidence-based resources in order to help your patients benefit from that knowledge will be useful in your career. One such resource is The National Center for Health Statistics. The Center has established Healthy People 2020 for tracking the nation’s health. In this assignment, you will begin from the topic you selected and propose a culturally competent, as defined by SAMHSA, community based health program.
Go to the Healthy People 2020 website and find the Topics & Objectives. Once you have located the 42 topic areas discussed on the website, choose a topic in which you are interested.
Write a 1,500-1,750-word paper in which you propose a community health program related to the topic you selected. Include the following in your proposal:
An overview of the program you are proposing. What is it? Who are the population(s) it will serve?
A rationale for the program.
A discussion of how the program will be established as and will remain culturally competent.

I’ve been a pharmacist for over 20 years, and in that time, I’ve seen firsthand how important it is to plan for the future. Whether you’re planning for retirement or just looking ahead at what’s next on your calendar, it’s important to consider how health issues might impact your life in the future. So let’s talk about some ways we can all start thinking about our health more seriously—and more consciously—over the next few years!
Find a doctor you trust.
Finding a doctor you trust is important. Your doctor should be someone who listens to your concerns, shares his or her knowledge and expertise, and has the time to address all of your questions. You may also want to consider asking for recommendations from friends or family members who have had similar experiences with their doctors.
When looking for a new primary care provider (PCP), do research on their qualifications, experience and training by visiting websites like Healthgrades or Vitalsource as well as checking out reviews posted in social media sites like Yelp or Facebook before making an appointment with them!
Develop a relationship with your pharmacist.
Pharmacists are a valuable resource for patients. They can be your first point of contact when it comes to managing medications and providing advice about how best to take them.
Pharmacists are also skilled at evaluating side effects of various drugs, so they can help you determine which ones are most likely to cause problems for you in the long term. For example, if your doctor prescribes an anti-anxiety medication called Xanax (alprazolam), your pharmacist might suggest that you keep an eye out for symptoms like dizziness or drowsiness while taking this drug—and let them know immediately if these occur so that they can adjust their dosage accordingly.
Get active in a way that works for your body and your lifestyle.

There are many types of exercise you can do that will work for your body and lifestyle.
Make mental health a priority.
It’s important to keep in mind that mental health is just as important as physical health. The National Institute of Mental Health estimates that 40 million adults in the United States suffer from a diagnosable mental illness, and nearly half of these individuals don’t receive treatment.
If you’re feeling down or depressed, there are many ways to improve your mental well-being:

Sleep more soundly—your brain needs time to rest and recharge while you’re asleep so it can function properly throughout the day.

Eat right—an unhealthy diet will make you feel sluggish and irritable, which means less energy for other activities like exercising or running errands around town (or even sitting still).

Exercise regularly—physical activity releases endorphins which make us happier because they help boost moods; studies have found that people who exercise regularly tend not only feel better physically but also emotionally too!
Set goals that feel challenging but attainable.
Now that you’ve got a general idea of what health goals are, it’s time to get specific. Here are some questions to ask yourself:

What do I want my body and mind to look like in five years? Ten years? The rest of my life?

Can I realistically achieve this goal within the next six months, year or two? If not, why not? Are there other things I can do now (or plan for) that will help me reach that goal later on down the road?

Sleep is a basic need for human health and well-being. The amount of sleep you need varies person to person, based on age, gender and other factors. However, the average adult needs seven hours of sleep each night for optimal health.*
The more vigilant you are in planning your nightly routine so that it includes some downtime before bedtime—like taking a bath or reading a book—the better off you’ll be when it comes time for bed.*
Examples: * Try eating dinner at around 6 p.m., then spend time relaxing until about 8 p.m.* When getting ready for bed (which should take no longer than 20 minutes), make sure everything is done before turning off lights and going downstairs; this will allow yourself plenty of time before falling asleep without having any distractions around while trying not fall asleep while waiting on those last few things anyway!
